Getting registered before your appointment helps us reserve the right amount of time for your care and keeps your first visit relaxed and unhurried. To register, simply contact our office or call us and our team will get your paperwork started.
What to Have Ready
To make your registration quick and easy, please have the following on hand:
- A photo ID and your dental insurance card, if you have coverage.
- A list of any medications you are currently taking.
- Any recent dental X-rays from a previous dentist, if applicable.
- Details of any medical conditions we should know about before treatment.
